1. Submit Valid U.S. Passport (photocopies are not acceptable) Be certain your passport is not excessively worn or damaged, If so, you may want to apply for a first time passport.

Download Passport Application (DS-4085)Note: This link will take you off our site to complete the form. Complete form online, when you are finished, you will see a "Create form " button, create the form, then "Print" the form. Once printed close that window to return to our site. Please print two copies
   
   
3. Download/Complete two Letters of Authorization
   
   
4. Download/Complete two-page Order Form
5. Copy of Travel Itinerary/Reservations
   (This is only needed for those who need Emergency 1-2 Day Service)
   Confirmed itinerary, copy of airline ticket, or a letter from your company printed on your stationery addressed to Washington passport agency introducing the applicant, stating the date and country of travel, requesting processing of additional visa pages prior to your travel date.

Important Notice: In case of an emergency, when your passport must be processed in 1-2 days, you must call our office for special instruction. Your Fed Ex package should be marked "FIRST AM DELIVERY". You must clearly indicate "DELIVER AFTER 7AM. - PLEASE SLIDE UNDER THE DOOR. Our building opens automatically at 7am; the Fed Ex driver will not be able to enter the building before that time
