Children Passport Guide



Please make sure you qualify to the following by checking each box before proceeding:

The Passport Applicant is under the age of 16 (15 years of age or less)

The Child applicant is a United States Citizen by birth, or Naturalization or has an Assignment of Citizenship by as U.S. Court or Magistracy.

You are applying with this applicant as (parent, listed on the Child's Certified copy of Birth Record) court ordered guardian or custodian of this Child Applicant.

If the answer to the above questions is "Yes" Then please continue to Step2: Collect Documents

Online Passport Application (DS-11) Note: This link will take you off our site to complete the form. Complete form online, when you are finished, you will see a "Create form " button, create the form, then "Print" the form. Once printed close that window to return to our site. Please print two copies.

Provide Proof of U.S. Citizenship (One of the below)

-Certified copy of Birth Certificate (with an state seal)

-Note: If your child has had a passport before you must submit both the birth certificate and the expired passport.

3) Proof of Identity(One of the below)

-Valid State photo I.D. or Drivers License

-Federally issued Photo I.D.

4) Provide Social Security Number of Child. (If you do not provide the Social Security Number the IRS may impose a $500.00 penalty fee).

5) One Passport Photograph

6) Download statement of Authorization (NOTICE FORM) need two forms with original signatures

7) Proof of travel I.e. airline itinerary/ticket or a hotel confirmation

8) Download Order Form for A-1 Passport/visa Express

9) Fees

Passport fees:

$140 + $25 = $165 for children under 16 years.

Service fees for A1 Passport & Visa Express:
$89 - Rush 5-10 business days service

$135 - Expedited 3-5 business days service
$179 - Emergency 1-2 business days Emergency Processing - By Appointment only

Fee Explanation:
You will be making payments to the U.S. passport agency, a federally authorized witnessing facility, and to our company.The passport fee must be paid by 2 checks or money orders. $25 witnessing fee payable to: Postmaster or clerk of the county court and $140 payable to: US. Department of State.

The fees for our services are to hand carry and submitting your passport documents to the passport agency and to pick up your passport when completed. This is the most assured way to guarantee the fastest possible issuance of your United States Passport.

Shipping Fees for return of your passport:
Please note: The shipping fee must be included with A-1 Passports fee. The shipping fee need not be taked to the post office when witnessing your signature

  • $23 - Fed Ex Letter (Priority Overnight – up to three passports delivered next day)
  • $26 – Fed Ex Letter (Priority Overnight – 3-5 passports delivered next day)
  • $38 – Fed Ex Letter (Saturday Delivery)

Please be advised that the shipping fee for some states are slightly higher than the ones mentioned above. Your receipt should indicate the fee differences also the above rates apply to most Continental USA. Higher rate applies to Alaska, Puerto Rico, Hawaii and US Virgin Islands.

 

NOTE: If the child is under the age of 14 then you must provide additionally:

  1. Provide evidence of the Child’s Relationship to the Parents or Guardian. Click Here for more information.

  3. In the event that one of the parents cannot be present at the passport acceptors window, the missing parent will need to complete and notarize Parental Consent Form DS3053. A copy of a drivers license must be attached to this form and presented to the passport acceptor.


In order to apply for an expedited U.S. passport you must first have your signature witnessed by an acceptable authority. Normally this is done by certain Post Offices and County Clerks.

Both parents must be present ALONG with your child during this process.

Find the Closest Passport Acceptance Facility

Before going be sure to have the following documents with you:

    1. Application Form DS -11 fully completed, but left unsigned

    2. Proof of Citizenship

    3. Proof of Identity from both parents

    4. Parental Consent Form - If one of the parents cannot go to the post office

    5. One recent Passport Photograph

    6. Letter of Authorization for A-1 Passport & Visa Express (Notice form)

    7. Proof of travel

    8. Payment for passport

IMPORTANT: Make copies of your Application

Bring all above original materials to a Passport Acceptance Facility and notify the agent you are using an expediting service to "hand carry" your application. Most facilities are open M-F, 10am to 4pm.

The acceptance agent will take about ten minutes to go through the following steps:

  • To verify your Child's identity, the agent will request your proof of Parentage. (Both parent’s must be present with Child, or one parent with notarized Form DS-3053 including his/her ID and Child present).
  • The agent will ask you BOTH parents (or absentee form DS-3053 and ONE parent) to sign the DS-11, and then will witness your signature and seal ALL documents in an envelope for Hand-Carrying.
  • To verify that you need an expedited passport, you must provide evidence of your departure date.
  • The agent will then seal the DS-11, photos, proof of citizenship, Letter of Authorization for A-1 Passport. Proof of departure date and the $112 check in an envelope marked "To be opened by U.S. Passport Office Only".

IMPORTANT: Do not open this sealed envelope or the application will be invalidated.

Please write the following on the outside of the sealed envelope:

1.Applicant's Name
2.Date of Birth
3.Departure Date

IMPORTANT: DO NOT SEAL THE ORDER FORM or payment for OUR services (A-1 Passport & Visa Express) inside the sealed envelope, they should be paper clipped outside of your sealed envelope along with extra copy of the applicatin DS11, extra letter of authoriztion (Notice form) with original signatures, copies of the parents ID, and copy of travel reservation

Place the following documents in a FedEx Priority Letter.

  1. Sealed Government Envelope
  2. A-1 Passport Order Form
  3. Payment to A-1 Passport & Visa Express - Credit Card, Company check, money order etc...
  4. Copies of application, extra letter of authoriztion (Notice form) with original signatures, copies of the parents ID, and copy of travel reservation

You must call us at 202- 639-1500 or 1-877-472-1500 and indicate you are processing an Expedited Passport from the Internet, or Fax Order Form to 202-639-9596

We recommend that you use FedEx as a safe means of overnight shipping. We will return your passport to you the same way.

Important Notice: In case of emergency, when your passport must be processed in 1-2 days, you must call our office for special instruction. Your Fed Ex package should be marked "FIRST AM DELIVERY". You must clearly indicate "DELIVER AFTER 7AM. - PLEASE SLIDE UNDER THE DOOR. Our building opens automatically at 7am; the Fed Ex driver will not be able to enter the building before that time. You must call our office to check availability of this service and follow the above instructions in order for us to receive your package to submit to the US Passport Agency before 9am.

Once you have done this we will process your request and ship your passport back to the address specified on the order form.
