Certified copy of legal document that caused the name change, e.g. a marriage certificate, divorce decree or court order. If you are using this service to correct your passport which may have been recently issued with errors such as name or date of birth, you must submit either a certified copy of your Birth Certificate or previously issued passport bearing the same credentials. Copies are not acceptable.

Confirmed itinerary, copy of airline ticket, or a letter from your company printed on your stationery addressed to Washington passport agency introducing the applicant, stating the date and country of travel, requesting issuance of a new passport prior to your travel date.

Important Notice: In case of emergency, when your passport must be processed in 1-2 days, you must call our office for special instruction. Your Fed Ex package should be marked "FIRST AM DELIVERY". You must clearly indicate "DELIVER AFTER 7AM. - PLEASE SLIDE UNDER THE DOOR. Our building opens automatically at 7am; the Fed Ex driver will not be able to enter the building before that time.
