You can only use this service
if you submit this request to the State Department within 365 days
(exactly) of your passport issuance date. If not, YOU MUST OBTAIN A NEW
PASSPORT THROUGH THE Renewal method.
Please make sure you qualify to the following by checking each box before proceeding:
You are in possession of a current U.S. passport that was issued within 12 months ago
You wish to extend the validity of or change the name on, or any other
information such as sex or date of birth.
If the answer to the above questions is "Yes" Then please continue to Step2: Collect Documents
Please make sure you collect all requirements before moving onto Step 3: Shipping
Submit Valid U.S. Passport
(photocopies are not acceptable) Be certain your passport is not
excessively worn or damaged, If so, you may want to apply for a first
time passport.

This link will take you off our site to complete the
form. Complete form online, when you are finished, you will see a
"Create form " button, create the form, then "Print" the
form. Once printed close that window to return to our
site. Please print two copies.
Certified copy of legal document
that caused the name change, e.g. a marriage certificate, divorce
decree or court order. If you are using this service to correct your
passport which may have been recently issued with errors such as name
or date of birth, you must submit either a certified copy of your Birth
Certificate or previously issued passport bearing the same credentials.
Copies are not acceptable.
Confirmed itinerary, copy of airline ticket, or a letter from your
company printed on your stationery addressed to Washington passport
agency introducing the applicant, stating the date and country of
travel, requesting issuance of a new passport prior to your travel
date.

$135- Expedited processing 3-5 business days service
$179- Emergency 1-2 business days service - By appointment only
Shipping Fees for return of your passport:
$23 - Fed Ex Letter (Priority Overnight–up to 3 passports delivered next day)
$26 – Fed Ex Letter (Priority Overnight – 3-5 passports delivered next day)
$38 – Fed Ex Letter (Saturday Delivery)
Please
be advised that the shipping fee for some states are slightly higher
than the ones mentioned above. Your receipt should indicate the fee
differences also the above rates apply to most Continental USA. Higher
rate applies to Alaska, Puerto Rico, Hawaii and US Virgin Islands.

Important Notice:
In case of an emergency, when your passport must be processed in 1-2
days, you must call our office for special instruction. Your Fed Ex
package should be marked "FIRST AM DELIVERY". You must clearly indicate "DELIVER AFTER 7AM. - PLEASE SLIDE UNDER THE DOOR. Our building opens automatically at 7am; the Fed Ex driver will not be able to enter the building before that time
