• Original, signed U.S. passport - must be valid for atleast 6 months containing at least 1 blank visa page.

• 1 Recent Passport type photos

• International Certificate of Vaccination for Yellow Fever
  
  
• Copy of round trip airline tickets or itinerary, or letter of confirmation from travel agent

Business Visa

• A company letter of intent and financial responsibility typed on the company’s stationery stating the following information(SAMPLE):
  
  
• Introducing the applicant, stating the applicant’s position with the company.
• Purpose of trip and a brief description of type of duties to be performed
• Name of company/contacts
• Financial responsibility for the applicant including return airfare. 
• A letter of invitation from host company or organization in Cambodia are sometimes required by the embassy, however this is not always required.

$55.00 per visa

Consular Fee: 
$100.00 Per Visa (Single Entry - Valid One Month)
$150.00 Per Visa (Multiple Entries - Valid Three Months)
$200.00 Per Visa (Multiple Entries - Valid Six Months)
$25.00 Per Visa (Additional Rush Fee for 24 hours Issue)

Please include a credit card authorization or a check/money order payable to: "A-1 Passport & Visa Express” for A-1 Handling Fee, Embassy Consular Fee, and return Shipping Fee.

Shipping Fees for return of your passport:

• $19.50 - Fed Ex Letter (Priority Overnight – up to three passports delivered next day)
• $23.50 – Fed Ex Letter (Priority Overnight – 3-5 passports delivered next day)
• $35.00 – Fed Ex Letter (Saturday Delivery)
