currently reside in Maryland, Virginia, West Virginia, or Washington DC.

- Original, signed U.S. passport - must be valid for at least 6 months containing and at least 1 blank visa page.
- 1 Application form fully completed and signed
- 1 Passport-type photo
- Copy of round trip airline tickets or itinerary
- Completed Order Form (print from browser)
- Valid US Green card or a valid US visa
- Letter of invitation establishing the purpose of trip
- Proof of sufficient funds
- Proof of lodging
- Copy of round trip airline tickets or itinerary
- Proof of travel health/accident insurance may be require

- $23 – Fed Ex Letter (Priority Overnight – up to three passports delivered next day)
- $26 – Fed Ex Letter (Priority Overnight – 3-5 passports delivered next day)
- $38 – Fed Ex Letter (Saturday Delivery)
