Tourist Visa
- Original, signed U.S. passport - must be valid for atleast 6 months containing at least 1 blank visa page.
- 1 fully completed and signed Visa Application Form.
- 1 Recent Passport type photo
- Completed Order Form (please print)
- International Certificate of Vaccination for Yellow Fever and
Cholera (recommended by the Embassy)
- Copy of round trip airline tickets
- Official letter from the bank certifying sufficient funds are available for the visit.
- Letter of invitation from Sierra Leone host
Business Visa
- Original, signed U.S. passport - must be valid for atleast 6 months containing at least 1 blank visa page.
- 1 fully completed and signed Visa Application Form.
- 1 Recent Passport type photo
- Completed Order Form (please print)
- International Certificate of Vaccination for Yellow Fever and Cholera (recommended by the Embassy)
- Copy of round trip airline tickets
- A company letter of intent and financial responsibility (Sample) typed on the company’s stationery stating the following information:
- Introducing the applicant, stating the applicant’s position with the company.
- Purpose of trip and a brief description of type of duties to be performed
- Name of company/contacts
- Financial responsibility for the applicant including return airfare.
- Please address your letter to:

Include
credit card authorization, a check or money order payable to "A-1
Passport & Visa Express " for A-1 handling fee per visa, plus
Embassy Consular Fee per visa, and return postage or Shipping Fee.
Shipping Fees for return of your passport:
- $23 – Fed Ex Letter (Priority Overnight – up to three passports delivered next day)
- $26 – Fed Ex Letter (Priority Overnight – 3-5 passports delivered next day)
- $38 – Fed Ex Letter (Saturday Delivery)
