- Original, signed U.S. passport - must be valid for atleast 6 months containing at least 1 blank visa page.
- 1 fully completed and signed Visa Application Form
- 1 Passport type photograph
- Copy of legal status in the US (i.e. Green card work authorization)
- Completed Order Form (A-1 request for passport/visa)
- International Certificate of Vaccination for Yellow Fever (not a copy)
- A company letter(Sample) of intent and financial responsibility typed on the company's stationery stating the following:
- Purpose and date of travel and duration of stay.
- Name and address of the contacts/company to be visited and nature of the business.
- Financial responsibility for the applicant as well as return transportation to U.S.
- This letter must be signed by a superior within your company other than the applicant
- Address the letter to:
- Embassy of Benin, Visa Section, Washington DC

Shipping Fees for return of your passport:
- $19.50 - Fed Ex Letter (Priority Overnight – up to three passports delivered next day)
- $23.50 – Fed Ex Letter (Priority Overnight – 3-5 passports delivered next day)
- $35.00 – Fed Ex Letter (Saturday Delivery)

Your documents will be hand-carried to the consulate and is usually processed in 48 hours. Validity of the visa will be 3 years for a period of 30 days each visit.
