page.
- 2 Visa Application Forms,
fully completed and signed
- 2 Passport-type photos
- Copy of round trip
airline tickets or itinerary, or letter of confirmation from travel
agent
- Completed Order Form (print from
browser)
- A company
letter of intent and financial responsibility (Sample)
typed on the company’s stationery stating the following information:
- Introducing the
applicant, stating the applicant’s position with the company.
- Purpose of trip and
a brief description of type of duties to be performed
- Name of
company/contacts
- Financial
responsibility for the applicant including return airfare.

$100.00 Per Visa - 5 years Multiple Entries
Include credit card authorization, a check or money order payable to
"A-1 Passport & Visa Express " for A-1 handling fee per visa,
plus
Embassy Consular Fee per visa, and return postage or Shipping Fee.
