completed and signed

- Completed Order Form (print from
browser)
- International
Certificate of Vaccination for Yellow Fever
- A company letter of
intent and financial responsibility typed on the company’s stationery
stating the following information(SAMPLE):
- The
letter must state all of the following:
- Introducing the
applicant, stating the applicant’s position with the company.
- Purpose of trip and
a brief description of type of duties to be performed
- Name of
company/contacts
- Financial
responsibility for the applicant including return airfare.
- The letter should be
signed by an officer of the company other than the applicant.
- Address this
letter to:
- Embassy
of Burundi, Visa Section, Washington D.C.
A-1 Service Fee:
$55.00 per visa
Consular Fee:
$ 80.00 Per visa - valid for stay of up to 30 days
$160.00 Per visa - valid for stay of more than 30 days
Include credit card authorization, a check or money order payable to
"A-1 Passport & Visa Express " for A-1 handling fee per visa,
plus Embassy Consular Fee per visa, and return postage or Shipping Fee.

Letter (Priority Overnight – up to three passports delivered next day)
- $26 – Fed Ex Letter
(Priority Overnight – 3-5 passports delivered next day)
