    • Copy of round-trip travel agency’s itinerary or tickets

    • Copy of Hotel Reservation or an invitation letter

Letter of consent for minor (under the age of 18)

A letter of consent notarized by parent(s) or guardian requesting issuance of a visa to a minor, stating the name of the person who will be responsible for the child while in Bolivia.

Certificate of vaccination

The original certificate of vaccination for yellow fever.

Bank Statement

A copy of recent bank statement showing sufficient funds. You may submit your regular checking, saving, or money market accounts or a letter from your bank printed on their stationery. You may erase your bank account number.

Certificate of vaccination

The original certificate of vaccination for yellow fever.

A1 Order Form

Company Letter

    • A company letter of intent and financial responsibility typed on the company’s stationery stating the following information (Sample):
      • Introducing the applicant, stating the applicant’s position with the company.
      • Purpose of trip and a brief description of type of duties to be performed
      • Name of company/contacts
      • Financial responsibility for the applicant including return airfare.
      • A letter of invitation from Host Company or organization in Afghanistan are sometimes required by the embassy, however this is not always required.
    • Please address your letter to:

Additional Requirements

Step 2: Fees

A-1 Service Fee:

$95.00 per visa

Consular Fee:

$160.00 For Multiple entry Tourist  or Business visa  (Approximately 10 business days)

A fee of $5 money order/check writing fee will be added to all visa fees.
