- If Tourist Travel: confirmation from hotel/tour operator, or letter of invitation from Czech citizen bearing the stamp of the Czech Immigration Authorities
- Evidence of Medical Insurance valid in the Czech Republic, to the value of US38,000
Business Visa
- Original, signed passport- must be valid for at least 6 months containing and at least 1 blank visa page.
- 1 fully completed and signed Application Form
- 2 Passport-type photos
- Copy of Green Card
- Completed Order Form (print from browser)
- Letter of invitation from host company in Czech Republic
- Evidence of Medical Insurance valid in the Czech Republic, to the value of US38,000

- $23.50 – Fed Ex Letter (Priority Overnight – 3-5 passports delivered next day)
