- Original, signed passport - must be valid for at least 6 months containing and at least 1 blank visa page.
- 1 Application form fully completed and signed
- 1 Passport-type photos
- Completed Order Form (print from browser)
- Copy of return/onward airline tickets
- Copy of U.S. Alien Registration (GREEN) Card if applicable
- Police clearance from district in which the applicant resides
A-1 Passport's Fees:
$55.00 per visa
Consular Fee:
$100.00 Per Visa - Single or Multiple Entries valid for 3 months
Please include a credit card authorization or a check/money order
payable to: "A-1 Passport & Visa Express” for A-1 Handling Fee,
Embassy Consular Fee, and return Shipping Fee.
Shipping Fees for return of your passport:
- $23 - Fed Ex Letter (Priority Overnight – up to three passports delivered next day)
- $26 – Fed Ex Letter (Priority Overnight – 3-5 passports delivered next day)
- $38 – Fed Ex Letter (Saturday Delivery)
