- Original, signed U.S. passport - must be valid for at least 6 months containing and at least 1 blank visa page.
- 1 Application form fully completed and signed
- 1 Passport-type photo
- Completed Order Form (print from browser)
- The name of an inviting individual or organization in Georgia and the exact dates of travel and purpose of travel are required in writing.
- Proof of legal U.S. visa status. (Green Card, or I-94 stapled into passport).
Business Visa
- Original, signed U.S. passport - must be valid for at least 6 months containing and at least 1 blank visa page.
- 1 Application form fully completed and signed
- 1 Passport-type photo
- Completed Order Form (print from browser)
- The name of an inviting organization in Georgia and the exact dates of travel and purpose of travel are required in writing on Company letterhead.
- Proof of legal U.S. visa status. (Green Card, or I-94 stapled into passport)
A-1 Passport's Fees:

Consular Fee:
Single Entry:
$10.00 Per visa/ up to 1 month stay
$30.00 Per visa/ up to 3 months stay
Double Entry:
$15.00 Per visa/ up to 1 month stay
$45.00 Per visa/ up to 3 months stay
Multiple Entries:
$100.00 Per visa/ up to one year
Please include a credit card authorization or a check/money order payable to: "A-1 Passport & Visa Express” for A-1 Handling Fee, Embassy Consular Fee, and return Shipping Fee.
Shipping Fees for return of your passport:
- $19.50 - Fed Ex Letter (Priority Overnight – up to three passports delivered next day)
- $23.50 – Fed Ex Letter (Priority Overnight – 3-5 passports delivered next day)
- $35.00 – Fed Ex Letter (Saturday Delivery)
