- Original, signed passport - must be valid for atleast 6 months containing at least 1 blank visa page.
- 2 fully completed and signed Visa Application Forms.
- 2 Recent Passport type photos
- Completed Order Form (please print)
- Letter of invitation from company to be visited in Malaysia(if business travel)
- Letter from employer as proof of employment
- Original confirmed round trip airline ticket in and out of Malaysia(flight itinerary)
- If permanent resident of U.S.A. - photocopy of Green card
- Expedite visa service available to residents of the states of:
- AL, FL, GA, IL, IN, KY, LA, MD, MS, NC, SC, OH, TN, TX, VA, WV, DC
A-1 Service Fee:
$55.00 per visa
Consular Fee:
$14.00 per Visa (Single Entry)
$27.00 per Visa (Multiple Entries)
Include
credit card authorization, a check or money order payable to "A-1
Passport & Visa Express " for A-1 handling fee per visa, plus
Embassy Consular Fee per visa, and return postage or Shipping Fee.
Shipping Fees for return of your passport:
- $23 – Fed Ex Letter (Priority Overnight – up to three passports delivered next day)
- $26 – Fed Ex Letter (Priority Overnight – 3-5 passports delivered next day)
- $38 – Fed Ex Letter (Saturday Delivery)
