U.S. Citizens do not require a visa for entry to this country. A valid U.S. Passport is required.
Tourist Visa
- Original, signed passport - must be valid for atleast 6 months containing at least 1 blank visa page.
- 2 fully completed and signed Visa Application Forms
- 2 Passport type photographs
- A photocopy of your flight Itinerary
- A
photocopy of your hotel confirmation for the duration of stay in Malta.
If staying with friends or relatives, an invitation from the hosts in
Malta is required.
- Proof of health insurance valid in Malta for the duration of stay.
- A photocopy of your Green Card, or proof of legal status in the USA
- Completed Order Form (A-1 request for passport/visa)
- Copy of recent bank statement or letter of reference from bank.
Business Visa
- Original, signed passport - must be valid for atleast 6 months containing at least 1 blank visa page.
- 2 fully completed and signed Visa Application Forms
- 2 Passport type photographs
- A photocopy of your flight Itinerary
- A photocopy of your hotel confirmation for the duration of stay in Malta.
- Proof of health insurance valid in Malta for the duration of stay.
- A photocopy of your Green Card, or proof of legal status in the USA
- An invitation from the host company in Malta \
- Completed Order Form (A-1 request for passport/visa)
- A company letter(Sample) of intent and financial responsibility typed on the company's stationery stating the following:
- Purpose and date of travel and duration of stay.
- Name and address of the contacts/company to be visited and nature of the business.
- Financial responsibility for the applicant as well as return transportation to U.S.
- This letter must be signed by a superior within your company other than the applicant
- Your letter must be addressed to:

Include
credit card authorization, a check or money order payable to "A-1
Passport & Visa Express " for A-1 handling fee per visa, plus
Embassy Consular Fee per visa, and return postage or Shipping Fee.
Shipping Fees for return of your passport:
- $23 – Fed Ex Letter (Priority Overnight – up to three passports delivered next day)
- $26 – Fed Ex Letter (Priority Overnight – 3-5 passports delivered next day)
- $38 – Fed Ex Letter (Saturday Delivery)
