- Original, signed passport - must be valid for at least 6 months containing and at least 1 blank visa page.
- 1 Application form fully completed and signed
- 1 Passport-type photo
- Completed Order Form (print from browser)
- Copy of round-trip airline tickets or airline itinerary
- Copy of Work Permit if applicable
- Copy of U.S. Alien Registration - Green Card

$55.00 per visa
Consular Fee:
$ 20.00 Per Visa - Transit Visa (stay up to max. 5 days)
$ 60.00 Per Visa - Single Entry Most Nationalities
$100.00 Per Visa - Multiple Entries
$ 35.00 Per Visa - Rush Fee, for 3 day processing
Include
credit card authorization, a check or money order payable to "A-1
Passport & Visa Express " for A-1 handling fee per visa, plus
Embassy Consular Fee per visa, and return postage or Shipping Fee.
Shipping Fees for return of your passport:
- $23 – Fed Ex Letter (Priority Overnight – up to three passports delivered next day)
- $26 – Fed Ex Letter (Priority Overnight – 3-5 passports delivered next day)
- $38 – Fed Ex Letter (Saturday Delivery)
