- Original, signed U.S. passport - must be valid for atleast 6 months containing at least 1 blank visa page.
- 1 fully completed and signed Visa Application Form (Cannot be Hand-Written)
- 1 Passport type photograph
- Copy of Travel Itinerary/Reservations
- Host
Company or establishment in Uzbekistan must also contact the Consular
Department of the Uzbek Ministry of Foreign Affairs to have an
invitation authorized and sent to the Embassy of the Republic of
Uzbekistan in Washington DC (and a copy sent to the traveler if
possible) on your behalf. The visa is not supplied until the approval
is got by the Embassy.

$ 55.00 Per Visa - Regular Issuance 7-10 business days.
$ 85.00 Per Visa - Rush Processing 4-5 days only if allowed by the embassy
Consular visa Fees(U.S. Citizens):
$140.00 - Validity is determined by the visa officer (issued in 7-10 days)
$210.00 - For Rush visa issued in 4-5 business days
Consular visa Fees(Non-US Citizens)
$ 40.00 - Up to one week stay
$ 50.00 - Up to 15 days stay
$ 60.00 - Up to one month stay
$ 80.00 - Up to three months stay
$150.00 - Multiple entries (up to six months)
$250.00 - Multiple entries (up to one year)

credit card authorization, a company check or money order payable to
"A-1 Passport & Visa Express" for A-1 handling fee per visa, plus
Consular Fee per visa, plus return Shipping Fee.
Shipping Fees for return of your passport:
- $23 - Fed Ex Letter (Priority Overnight – up to three passports delivered next day)
- $26 – Fed Ex Letter (Priority Overnight – 3-5 passports delivered next day)
- $35 – Fed Ex Letter (Saturday Delivery)
Please
be advised that the shipping fee for some states are slightly higher
than the ones mentioned above. Your receipt should indicate the fee
differences also the above rates apply to most Continental USA. Higher
rate applies to Alaska, Puerto Rico, Hawaii and US Virgin Islands.
