Tourist Visa

Passport

• • Original, signed U.S. passport, valid for at least 6 months and containing at least 1 blank visa page.

Application

• • 1 Application Form, fully completed, signed.
  • A valid email address is required for each application. – “Humanity validation” (CAPTCHA) may require multiple attempts. Keep refreshing to clear the validation.
  • There is no space for a signature. After printing the form, applicants must sign at the bottom of the page.

Proof of Travel

• • Copy of travel tickets or itinerary, showing round trip travel

Photos

• • 2 Passport type photographs

A1 Order Form

• • Completed two-page Order Form

Additional Requirements

• Tourist voucher from a hotel in Azerbaijan – Must have hotel confirmation number. OR for friend/relative visitor visas: An invitation Letter from Azerbaijani citizen or foreign citizen currently residing in Azerbaijan along with their Azerbaijani ID.

Business Visa

Passport

• • A letter of invitation from the inviting organization authorized by the Azerbaijan Ministry of Foreign Affairs

Consular Fee:

$25.00 per visa – 3 Months Single Entry Tourist
$355.00 per visa – Business: The Embassy’s choice of a one year multiple entry or 3 month single entry
