Tourist Visa

Passport

• • Original, signed U.S. passport – must be valid for at least 6 months containing at least 1 blank visa page.

Application

• • 1 Application Form, fully completed, signed, and notarized

Proof of Travel

• • Copy of round trip airline tickets or itinerary, or letter of confirmation from travel agent

Photo

• • 1 Recent Passport type photo

A1 Order Form

• Completed two-page Order Form

Business Visa

Passport

Invitation Letter

• • A letter of invitation from host company or organization in Cambodia are sometimes required by the embassy, however this is not always required.

Photo

• • 1 Recent Passport type photo

A1 Order Form

• • Completed two-page Order Form

Company Letter

• A company letter of intent and financial responsibility typed on the company’s stationery stating the following information (Sample):
  • Introducing the applicant, stating the applicant’s position with the company.
  • Purpose of trip and a brief description of type of duties to be performed
  • Name of company/contacts
  • Financial responsibility for the applicant including return airfare.

A-1 Service Fee:

$95.00 per visa

Consular Fee:

Single entry visa $60.00  for stay up to 30 days, valid for 3 months

Multiple entry visa $114 for stay up to 30days, valid for 5 years

A fee of $5 money order/check fee will be added to all visa fees.

Please note that the above rates apply to most Continental USA. Higher rate applies to Alaska, Puerto Rico, Hawaii and US Virgin Islands.
