Passport

• • Copy of round trip airline tickets, e-tickets, or itinerary

• • 2 Recent Passport type photos

A1 Order Form

• • Completed two-page Order Form

Additional Requirements

• Letter from employer verifying employment or leave of absence
• Letter explaining purpose of trip to Indonesia
• Copy of most recent bank statement required

Social Visit Visa

Passport

Proof of Travel

• • Copy of round trip airline tickets, e-tickets, or itinerary

Photos

• • 2 Recent Passport type photos

A1 Order Form

• • Completed two-page Order Form

Additional Requirements

• Letter explaining purpose of trip to Indonesia to the embassy
• Letter of invitation from host family / friends
• Proof of funds (bank statements)

Business Visa

Passport

• • Original, signed U.S. passport – must be valid for at least 6 months containing at least 1 blank visa page.

Applications

• • 2 Application Forms, fully completed and signed.

Proof of Travel

• • Copy of round trip airline tickets or itinerary

Proof of Stay

• • If a multiple entries visa is required, an invitation from the host organization in Indonesia is required

Invitation Letter

• • A letter of invitation from the host, ONLY if applying for multiple entries

Photos

• • 2 Recent Passport type photos

A1 Order Form

• • Completed two-page Order Form

Company Letter

• A company letter of intent and financial responsibility (Sample) typed on the company’s stationery stating the following information:
  • Introducing the applicant, stating the applicant’s position with the company.
  • Purpose of trip and a brief description of type of duties to be performed
  • Name of company/contacts
  • Financial responsibility for the applicant including return airfare.
  • The letter should be signed by an officer of the company other than the applicant.
• Please address your letter to:

A-1 Service Fee:

$95.00 per visa (Regular)

Consular Fee:

A $5 money order/check fee will be added to all visa fees.

Please note that the above rates apply to most Continental USA. Higher rate applies to Alaska, Puerto Rico, Hawaii and US Virgin Islands. Please note all FedEx envelopes are shipped without a signature requirement, unless requested. A small fee will apply for a signature requirement.
