Tourist Visa

Passport

• • Original, signed U.S. passport – must be valid for at least 6 months containing at least 1 blank visa page.

Application

• • 2 copies –  Application Form, fully completed, signed and dated.

Invitation Letter

• • 2 copies – Letter of invitation if visiting friends or family / Hotel confirmation if staying at a hotel

Photos

• • 2 Passport-type photographs

A1 Order Form

• Completed two-page Order Form

Business Visa

Passport

• • 2 copies of Application Form, fully completed, signed.

• • 2 copies of invitation from the host organization in Pakistan should be provided

Photos

• • 2 Passport-type photographs

A1 Order Form

• • Completed two-page Order Form

Company Letter

• • 2 copes of a company letter of intent and financial responsibility (Sample) typed on the company’s stationery stating the following information:
    • Introducing the applicant, stating the applicant’s position with the company.
    • Purpose of trip and a brief description of type of duties to be performed
    • Name of company/contacts
    • Financial responsibility for the applicant including return airfare.

    • Your documents will be hand-carried to the consulate/embassy and is usually processed in 1-4 weeks. The visa processing time is at the embassy’s discretion.

Note: Members of the news media must also complete and submit 4 copies of the  FORM-A

A-1 Service Fee:

$150.00 per visa

Consular Fee:

$192.00 per Tourist Visa  – Single or Multiple Entries
$324.00 per Business Visa – Single or Multiple Entries

A fee of $5 money order/check writing fee will be added to the above consular visa fee.

Return Shipping Fees

Please be advised that the shipping fee for some states are slightly higher than the ones mentioned above. Your receipt should indicate the fee differences also the above rates apply to most Continental USA. Higher rate applies to Alaska, Puerto Rico, Hawaii and US Virgin Islands.

Calculate: Consular Fee + A1 Service Fee + Shipping = Total Due
