Passport

• Original, signed passport – must be valid for atleast 6 months containing at least two blank visa pages.

Application

• 1 Application Form, fully completed, signed, and notarized

Proof of Travel

• Copy of travel airline itinerary or air tickets that show round trip transportation

Proof of Stay

• Copy of hotel confirmation(s) for the length of intended stay

Photos

• 2 Passport type photographs

A1 Order Form

• Completed two-page Order Form

Additional Requirements

• Copy of U.S. Alien Registration or proof of legal residence in the U.S.A.(valid U.S. visa)
• Copy of current Medical Insurance Policy showing coverage
• Copy of most recent bank statement
• Yellow Fever certificate if arriving from, or traveling to, or through, the yellow fever endemic area

Business Visa

Passport

Invitation Letter

• Invitation from the company or organization in South Africa (required at the discretion of the consular)

Photos

• 2 Passport type photographs

A1 Order Form

• Completed two-page Order Form

Company Letter

• A company letter (Sample) of intent and financial responsibility typed on the company’s stationery stating the following:
  • Purpose and date of travel and duration of stay.
  • Name and address of the contacts/company to be visited and nature of the business.
  • Financial responsibility for the applicant as well as return transportation to U.S.
  • This letter must be signed by a superior within your company other than the applicant
• Please address your letter to:
